Fill out and return to Fox’s Pizza Den (Punta Gorda) no later than Saturday December 26th.

What to Do:

1. Fill in your name, phone number, and email address.
2. Circle one team from each game.
3. Write in total combined points you predict will be scored for the Monday Night game.
4. Turn game card in by date and time indicated on the top of game card.
5. Entry with most correct teams for the week will win a Big Daddy Pizza (21 pieces).
6. In the event of two or more entries having the same amount of correct answers,
   the winner will be determined with the combined score for the noted Monday Night game.
   The closest to the actual total wins.
7. Winners will be notified by phone on Tuesday.
